SCAHP Foundation Community Outreach and Education

SCAHP Foundation is a 501(c) 3 tax exempt organization that plays a leadership role in empowering citizens across South Carolina to lead healthier lives through disease management education and prevention. SCAHP Foundation partners with like minded health care agencies and groups across the state to hold conferences, health fairs and other medical education events. SCAHP Foundation initiates and participates in many inter-industry collaborate projects such as creating a uniform provider credentialing form and providing provider education programs targeted towards best practices in disease management. SCAHP Foundation believes that collaboration with providers is important to ensuring that citizens receive high quality and affordable health care.

Over the years our programs have included initiatives on diabetes, obesity, kidney disease, asthma, cardiovascular disease, hypertension, school based health programs, individual wellness programs, and nutrition. We have partnered with the South Carolina Department of Health and Human Services, the South Carolina Department of Health and Environmental Control, the South Carolina Association of School Nurses, the Medical University of South Carolina, the University of South Carolina School of Medicine, the South Carolina Chapter of the American Diabetes Association and the National Kidney Foundation.
