Community Health Initiatives
SCAHP is a community leader in disease management and prevention in South Carolina. We are devoted to collaborating with other entities to improve the overall health of citizens through community health initiatives. For nearly ten years, SCAHP has focused on community health initiatives as a means to educate community members on various chronic diseases and how to best manage, as well as prevent, the progression of disease states. All of the SCAHP's community health initiatives comply with standards of care put forth by national organizations. Our mission is to educate and empower South Carolinians to live healthier lives. Our programs have focused on diabetes, obesity, kidney disease, asthma, cardiovascular disease, hypertension and hyperlipidemia.The success of the SCAHP's community health initiatives lies in collaboration. Since sponsoring our first program in 1998, SCAHP has successfully partnered with the South Carolina Association of School Nurses, the Medical University of South Carolina, the University of South Carolina School of Medicine, the Department of Health and Environmental Control, the South Carolina Department of Health and Human Services, the South Carolina Association of School Nurses, the South Carolina Chapter of the American Diabetes Association, and the National Kidney Foundation of South Carolina. In addition, we partner with many community health organizations across the state, such as, the Charleston County Diabetes Coalition, and Family Connections, a nonprofit organization that helps parents of children with disabilities locate support resources. We also work with local healthcare providers and churches. SCAHP has become a valued and responsive partner for local and statewide health initiatives.
